This is no longer a fringe strategy. It's disciplined purchasing.

Nevada's provider market concentration magnifies every issue discussed here. Here's what employers in this state should be doing now:

1. Calculate Your Commercial-to-Medicare Multiple

If you don't know your average reimbursement as a percentage of Medicare, you're operating without a baseline. In consolidated markets, that figure can exceed 300% of Medicare. That level of variance is not defensible long-term. Demand a repricing analysis of your claims.

2. Question the Assumption That Bigger Networks Equal Better Value

Broad PPO networks often mean higher unit costs, minimal steerage, and limited price discipline. Network breadth is not a value strategy — it's a convenience strategy. If a hospital system knows it cannot be excluded, it has no incentive to moderate pricing.

3. Scrutinize Incentives

Employers should clearly understand broker compensation structures, carrier performance guarantees, and bonus arrangements tied to premium growth or volume. Healthcare procurement deserves the same rigor as any other major vendor relationship.

4. Explore Medicare-Anchored or Value-Based Designs

Reference-based pricing (e.g., Medicare + X%) is becoming mainstream among employers who want price predictability tied to an objective benchmark. In markets like Las Vegas or Reno, anchoring payment methodology can materially reset negotiations — if implemented thoughtfully with member support.

5. Use Steerage With Intent

Transparency does not change behavior on its own. Employers can waive cost-sharing for high-value providers, incentivize use of bundled surgical programs, offer navigation support, and design benefit differentials that move volume. Hospitals respond when volume shifts. Without steerage, price power remains intact.
